Saw Palmetto (Serenoa repens)
is an extract of the pulp and seeds of a dwarf, scrubby palm tree native
to the west Indies and the Atlantic coast of the United States. It is
generally accepted that Saw Palmetto works, at lest in part, by the same
major mechanism as finasteride (PROSCAR), i.e., preventing the
conversion of testosterone to DHT. However, Saw Palmetto not only lowers
the rate of DHT formation, but blocks the ability of DHT to bind to
cells, preventing the action of hormone on receptors. In addition, it
may prevent severe inflammatory responses via a dose related effect on
the arachidonic acid cascade through a double blocking of
cyclo-oxygenase and lipoxygenase pathways. In one study examining 110
subjects, it decreased night time urination by 45%, increased urinary
flow rate more than 50%, and reduced the amount of urine left in the
bladder after urination by 42%. In other large trials, improvement in
prostatic symptomatology was readily noted and saw palmetto even
compared favorable with Hytric (Alpha blocker) and/or Proscar (finasteride)
in affecting the symptomatology of BPH when these agents were compared
head to head.
